CERTIFICATION REQUEST
1. Condiciones de los Solicitantes
Applicants Conditions
• El aspirante debe manifestar cualquier necesidad especial para la certificación (idioma, alimentación, etc), el Organismo de Certificación de Personas dará respuesta durante los tres (3) días hábiles después de recibida.
The applicant must state any special need for certification (language, food, etc.), the Personnel Certification Body will respond within three (3) business days after receipt.
• Declaro que conozco, entiendo y acepto las condiciones de certificación especificadas en el MN-RH-001 Manual de Funciones, Derechos y Deberes de los aspirantes
I declare that I know, understand and accept the certification conditions specified in the MN-RH-001 Manual of Functions, Rights and Duties of the applicants.
• Declaro que me presento voluntariamente al proceso de certificación y me comprometo a cumplir los requisitos establecidos, así como suministrar cualquier información necesaria para la evaluación, a no tomar parte en prácticas fraudulentas, ni divulgar materiales utilizados en el desarrollo de mi certificación.
I declare that I voluntarily submit to the certification process and I agree to comply with the established requirements, as well as provide any information necessary for the evaluation, not to take part in fraudulent practices, or disclose materials used in the development of my certification.
• Autorizo a entregar la información del proceso de certificación cuando sea solicitado por autoridad competente

• El Organismo de Certificación de Personas CONSEJO COLOMBIANO DE CERTIFICACIÓN – CCC de la sigla INTERNATIONAL SECURITY CERTIFICATION BOARD - ISCB, dando cumplimiento a la ley 1581 de octubre 17 de 2012 sobre protección de datos personales, informa que los datos incluidos en este documento; serán custodiados, conservados y utilizados exclusivamente para la entrega de beneficios y envío de información de servicios, quedando autorizado con la respectiva firma o envío en forma digital del usuario en forma libre, expresa y voluntaria a realizar el tratamiento de sus datos. Lo anterior no impide al ejercicio de los derechos al acceso, rectificación, actualización y supresión de los datos personales en cualquier momento.

• En el caso de existir algún tipo de impedimento o situación que no permita el acceso del aspirante al proceso de certificación, autorizo a que este sea comunicado por correo electrónico

• Si el solicitante es aceptado en el proceso y cumple con las etapas definidas y los requisitos de certificación, recibirá la correspondiente certificación en la competencia laboral por parte del Organismo de Certificación de Personas; dicho certificado es de propiedad exclusiva del Organismo de Certificación y una vez expedido hará parte de una base de datos que estará disponible en la página web del Organismo de Certificación y es de consulta pública
If the applicant is accepted in the process and meets the defined stages and certification requirements, he will receive the corresponding certification in labor competence from the Personnel Certification Body; Said certificate is the exclusive property of the Certification Body and once issued it will be part of a database that will be available on the Certification Body's website and is for public consultation.
2. Condiciones Obligatorias de Seguridad para Procesos de Evaluación
Mandatory Security Conditions for the Evaluation Process
• No presentarse bajo efectos de sustancias psicoactivas o alcohol.
Do not show up under the influence of psychoactive substances or alcohol.
• Se debe presentar personalmente a realizar las pruebas con documento de identificación.
You must present yourself in person to carry out the tests with an identification document.
• Saber leer y escribir.
Know how to read and write.
• Si usted usa lentes, audífonos o prótesis de algún tipo y debe usarlas durante el desarrollo de las pruebas para asegurar su mejor desempeño en las mismas, así mismo si sufre de alguna enfermedad diagnosticada, debe presentar los soportes y un informe favorable de su médico sobre el control de dicha enfermedad.
If you wear glasses, hearing aids or prostheses of any kind and you must use them during the development of the tests to ensure your best performance in them, likewise if you suffer from a diagnosed disease, you must present the supports and a favorable report from your doctor on the control of said disease.
• Está prohibido el uso de cámaras o demás herramientas para ayudar en el examen.

• No es permitido hacer uso del celular durante el proceso de evaluación.
It is not allowed to use the cell phone during the evaluation process.
• El candidato deberá contar con una cámara externa conectada al computador, la cual estará encendida durante todo el proceso de evaluación.

• Deberá tener instalada la aplicación de ZOOM en el computador.

• Deberá compartir pantalla, tener el audio y la cámara encendida durante todo el proceso de evaluación.

• Deberá seguir las instrucciones que imparta el par evaluador.
You must follow the instructions given by the peer evaluator.
El incumplimiento de dichas condiciones dará lugar a la suspensión del proceso de evaluación y posterior anulación del proceso de certificación si se considera pertinente.
Failure to comply with these conditions will lead to the suspension of the evaluation process and subsequent cancellation of the certification process if it is considered pertinent.
Nota 1: Adjuntar copia del documento de identidad, así como de los documentos que soporten los datos consignados en este formulario (ultimo nivel de estudios y experiencia laboral), antecedentes disciplinarios.

Nota 2: El servicio de certificación, es un proceso voluntario que adelanta el solicitante y que es concertado con el Organismo de Certificación de Personas, en el cual el solicitante debe demostrar que, en su desempeño laboral, cumple con los estándares definidos en las Normas de Competencia Laboral en las cuales aspira certificarse.
